Waste Data Sheet

While supplying wastes to our Company please use the attached form of the Waste Data Sheet.

Explanation:
a – a number given to the waste by its owner who delivers the given batch of waste.
b - name and surname or the name of the company.
c - in case of municipal wastes the data sheet to be filled out by the company’s representative who had got the appropriate permit in the field of wastes collection from fixed properties owners under the Municipal Order Keeping Act of  September 13, 1996. (Journal of Acts No. 236, item 2008), or to be filled out by the municipal organizational entity under the same Act.
d - In case the waste is transported through two or more business entities dealing with wastes transportation you should enter the required data and provide signatures in the marked fields of all business entities dealing with transportation of wastes paying attention to the  order / sequence of the waste transportation.
e – company’s seat address or place of residence.
f - the place of wastes receipt where the waste is to be supplied, indicated by the owner of the waste dealing with wastes transportation.
g - In case of hazardous wastes the date of delivery of such waste is to be provided. The Waste Data Sheet can be used as a single sheet or a collective sheet, describing a given type of waste delivered collectively during a calendar month, through the same entrepreneur dealing with wastes transportation and supplying the wastes to the same owner of wastes.
h – enter the volume of wastes at least exact to one decimal place with regard to wastes other than hazardous ones and exact at least to three decimal places in case of hazardous wastes.
i – relates to hazardous wastes.
